Online Service Order Form

Why this Form?

You can use this form if:

• You are bringing your computer to MBS for Repair Service (it will save you several minutes of manual form-filling).
• You are using a courier service to drop off a unit for Repair Service.

The online form will guide you through the submission process:

• Reading and accepting our Terms and Conditions.
• Optionally check your Apple Computer's Warranty Status.
• Choosing the most adequate service option for you.
• Filling out your contact information.
• Entering your computer's model and serial number.
• Describing the computer's problem.

After you enter all the required information,
the form will print:

• 2 copies of the information you provided  (keep one with you, sign & date the the other one at bottom of the page, and send it with your computer)
• 1 copy of out Terms and Conditions (keep it with you)
